Open up the invitation that you received from your medical organization and click on the link to begin.
If you already have an existing FollowMyHealth account, select the "Sign in and add this connection" option.
This will then give you the option of selecting the login method for your existing account. If you have an FMH Secure username/password, select the FMH Secure icon. This looks like a green shield (A).
If you use an alternative login method (Facebook, Google, Yahoo, etc.), then select "or, use an alternative" (B1) and choose the appropriate icon for your specific login credentials (B2)
Input your username/password for your existing FollowMyHealth portal. If the login is successful, you will be presented with a series of 4 steps to create the connection to your medical organization. In step 3 (step 2 if you are adding this connection), you will be prompted to enter an invitation code, as seen in this picture.
By default, the invite code is either your 4-digit year of birth (just the year, not the full birthdate), or the last four digits of your Social Security Number. Your medical organization may have communicated their unique approach in the invitation email. If you do not have your SSN (or other identifying information) on file with the organization, you will need to contact them to learn of your specialized invitation code.
Once your invitation code has been accepted you will be asked to accept the Release of Information. This agreement is similar to what you would sign if you requested a paper copy of your records from the doctor's office. It will be available in the "Documents" section of "My Health" if you should like to reference it later.
Please Note: If you select "I Decline", the FollowMyHealth patient portal creation process will not be completed.
After you have accepted the Release of Information, you should see an animation that resembles the following:
During this step, your health records from your healthcare organization will be uploaded into your Personal Health Record. Once the upload has been completed, you will be brought directly into your online patient portal, and your medical information should be available for your viewing.
