How do I Become an Authorized User/Proxy for my Minor Child?

Below are the steps for accepting an authorized user/proxy invitation to gain access to a minor's records. If you do not have a proxy invitation, click here to learn how to obtain one.

As a proxy to the minor, you will need to create/connect the minor's records to your own "parent" or "host" Universal Health Record (UHR).

  • You do not have to be connected to a medical organization, but you will need your own login for FollowMyHealth®. You will be offered the opportunity to create one while accepting the invitation.
  • If you already have a FollowMyHealth UHR, you are ready to connect as a proxy!
  When the proxy invitation arrives, please click the included link, and the following screen will appear:

If you DO NOT yet have your own FollowMyHealth UHR, select "Sign Up and Connect".

Otherwise, select "Sign in and add this connection" and log in with YOUR FollowMyHealth username/password.

NOTE: Selecting "Sign Up and Connect" will bring up the following screen, where you will need to provide YOUR demographic information, not the minor's. This is in preparation for creating the "host" or "parent" account mentioned earlier.  Once filled, select "Confirm and Continue"

Read through and accept the Terms of Use by clicking "I Accept' at the bottom of the window
Note: Selecting "I Decline" will not allow the process to complete

You will then be asked to create an FMH Secure username and password. For more details on that, please click here.

After you have successfully set up a username and password, you will be presented with the following six steps to register as a proxy.

Step 1: Welcome - Lets you know about the account creation. Click "Next" to continue.

Step 2: Enter Invite Code - Enter the invite code that was communicated to you.  If you do not know what it is, please consult the invitation email or contact the organization.  After entering the Invite code, click on "Next"

Step 3: Release of Information - The Release of Information must be agreed to in order to connect to your medical organization through FollowMyHealth. This is similar to what you would sign when picking up a paper copy of your record. This document will be available at any time in the "Documents" section of your FollowMyHealth account. For more information, see our security articles. Click "I Agree" If you agree to the Release of Information agreement and would like to connect to your medical organization through FollowMyHealth. If you click "I Decline", your FollowMyHealth account will still be created, but it will not be connected to your child's medical information.

Step 4: Authorized Individual Acceptance - This says that you are being granted access to this child's medical information through FollowMyHealth, and you are verifying that you have the right to have access to this information. Click "I Accept" if you agree; clicking "I Decline" will not allow you to access your child's medical information.

Step 5: Authorized Individual Authorization - You are accepting, on behalf of the child, to grant access for yourself to the child's medical records. Click "I Accept" if you agree; clicking "I Decline" will not allow you to access your child's medical information.

Step 6: Upload Health Record - The medical information will then begin to upload to the UHR. Once the upload has been completed, you will be brought directly to the Home page of the UHR.

The minor’s information will then be accessible by choosing their name from the drop down menu in the upper-right corner (where your name is first shown).  Select the black arrow to the right of your name.  When the drop down menu populates, select your child and this will allow you to toggle to their UHR.

It is important to remember that once the child reaches the protected age you will no longer receive updates or notifications to their medical record. You will only be able to view historical information. Click here to read more.
