Why is my Pharmacy Missing or Outdated?

At this time, all pharmacy updates in the FollowMyHealth™ patient portal are manually resolved by our FollowMyHealth™ Database Administrator. If you cannot find your preferred pharmacy in the pharmacy search; or if you find an outdated pharmacy that needs edited or deleted, please fill out the below template to the best of your knowledge and send to technical support:

  • Zip
  • Phone #
  • Fax #
  • NCPDP #
