Update Personal Info/Demographics on a Mobile Device

From Demographics, you can manage your basic information, emergency contact information, insurance information, and information about the person who is responsible for your account.  You can access your demographic by tapping the More icon in the bottom right and select My Account



Select which tab under Demographics that needs to be changed:

General - section contains General Personal Information including name, Date of birth, Social Security Number, gender, address, phone numbers, and social information (Race/Ethnicity)

Emergency Contact - information about a person to contact in case of an emergency, on your behalf. 

Responsible Party - information about who is responsible for any charges incurred with the healthcare organization.

Insurance - Lists your Primary, Secondary, and Tertiary insurance policies.  Select one of these items to edit it. 

Preferred Pharmacy - Lists your current pharmacy connected to your portal.  Select to change/edit this information. 

...and then choose Edit in the top right:



After reviewing/editing the appropriate demographic information, be sure to select "SAVE" in the "Edit General" menu.   



NOTE 1:  If you attempt to exit the Demographics page without saving the changes, you are asked to confirm the cancel, or continue editing.  If you decline and tap Yes, Discard Changes, your changes are not saved and you are returned to the previous demographic screen.  If you tap No, Continue Editing, you return to the section you are editing.  Then be sure to hit Save in the top right.

NOTE 2:  If you are connected to any organizations that accept changes to your demographic info, you will be asked if you want to send these updates to them.  If this option does not appear, it is recommended to contact your medical providers directly to let them know of the updates to your information. 

Android App:

Insurance - Lists your Primary, Secondary, and Tertiary insurance information.  Includes details on your basic policy, your employer name and address, and policy holder information

To update your demographics, just click on "Edit" beside the section of interest. After reviewing/editing the appropriate demographic information, be sure to select "SAVE DEMOGRAPHIC CHANGES" at the bottom of the screen. 



NOTE 1:  If you attempt to exit the Demographics page without saving the changes, you are asked to do so.  If you decline and tap No, your changes are not saved and you are returned to My Account.  If you tap Yes, your changes are saved, a success message is displayed, and you'll remain on the Demographics screen

NOTE 2:  If you are connected to any organizations that accept changes to your demographic info, you will be asked if you want to send these updates to them, after selecting the "DONE" option.  If this option does not appear, it is recommended to contact your medical providers directly to let them know of the updates to your information.
