How to Remove a Proxy Connection/Account

In your FollowMyHealth™ account, you can remove individuals that you have access to. To remove an individual, follow the steps below.

1. Log into the account

2. Go to My Account and select My Connections



3. Click on ​"With Family Members, Dependents, etc." and you will see the list of individuals you have access to



4. Once you find the person you wish to remove, click on Remove to the right. You will receive a message about being sure you want to remove; click Yes. 



Note: You cannot remove a proxy connection/account from the mobile app.
