How to Connect to a Health Source?

Connect an iHealth health source
Use the iHealth® health source to access weight, blood pressure, and blood glucose measurements
from your patient portal account.

Before you begin
You must purchase an iHealth® device. Click Visit product website from the iHealth® application
in Devices&Apps to be directed to the iHealth® Store.
1. From the patient portal, click My Account > My Connections.
My Connections opens.
2. Click the With Devices and Apps through Health Sources tab.
The With Devices and Apps through Health Sources tab is displayed and you can view
your connected health sources and available health sources.
3. From Available Health Sources, find the iHealth® health source and click Add Connection.
iHealth Labs Login opens.
Follow the instructions from the iHealth® web page to complete the connection.

Results of this task
The iHealth® health source is connected to FollowMyHealth®.
The iHealth® health source is now listed under Your Connected Health Sources.
 

Connect a Nokia health source
The Nokia health source provides a link between FollowMyHealth® and your personal Nokia
device. You can measure your vital signs and the data automatically uploads to the patient portal.

Before you begin
You must purchase a Nokia device. Click Visit product website from the Nokia health
source in Devices&Apps to be directed to the Nokia Store.
1. From the patient portal, click My Account > My Connections.
My Connections opens.
2. Click the With Devices and Apps through Health Sources tab.
The With Devices and Apps through Health Sources is displayed and you can view your
connected health sources and available health sources.
3. From Available Health Sources, find the Nokia health source and click Add Connection.
My Nokia account opens.
If you do not have an account, click Don't have an account yet. Account Creation opens
and you can create an account.
4. Sign in with the user name and password that you created when you created your Nokia
Authorize FollowMyHealth opens.
5. Follow the steps provided by the Nokia web page to connect your device.

Results of this task
The Nokia health source is connected to FollowMyHealth®.
The Nokia health source is now listed under Your Connected Health Sources.


Connect a Fitbit health source
The Fitbit health source provides a link between FollowMyHealth and your personal Fitbit device.
You can use your Fitbit device to count your daily steps and the data automatically uploads to the
patient portal.

Before you begin
You must purchase a Fitbit device. Click Visit product website from the Fitbit health source in
Devices & Apps to be directed to the Fitbit store.
1. Sign in to the patient portal.
2. From the static toolbar, click My Account > Connections.
Connections opens.
3. Click the With Devices and Apps through Health Sources tab.
The With Devices and Apps through Health Sources is displayed and you can view your
connected devices and applications and available health sources.
4. From Available Health Sources, find the Fitbit health source and click Add Connection.
My Fitbit account opens.
If you do not have an account, click Don't have an account yet. Account Creation is displayed
and you can create an account.
5. Sign in with the user name and password that you created when you created your Fitbit account.
Authorize FollowMyHealth opens.
6. Follow the steps provided by the Fitbit web page to connect your device.

The Garmin health source provides a link between FollowMyHealth and your personal Garmin device.

Devices & Apps to be directed to the Garmin store.
