How do I add a Document on Mobile? (iOS)

You can add  a document to your health record from your mobile device.

1. From Home, tap "My Health"

    My Health opens.





2. From My Health, tap DocumentsDocuments opens.




3. Tap the "+" icon.

     Confirm Add opens.




Note: If you do not want to confirm every document that you add to your health record, tap Do not show this message again, otherwise tap OK.

Add opens.





4. Tap Name to open the on-screen keyboard and enter the name of the document.
5. Tap Done.  The name of the document is displayed.
6. (Optional) Tap Date to enter the document date. A calendar date selector is displayed.





7. (Optional) Select the date and tap Done.
8. (Optional) For Provider, enter the provider associated with the document.
9. Tap Add Item.
10.Tap Select Image to take a photo of the document being added or to select an image form your photo library.  Documents can have 1 or more pages.

>Tap Take Photo to use your mobile device to take a photo of the document to upload. Tap OK to enable FollowMyHealth Mobile to access your camera.  After you take the photo, tap Use Photo and then tap Save.

>Tap Choose from Library if the document image already exist in your photo gallery on your mobile device.  Photos opens.  Tap the image that you want to upload.  Tap Choose and then tap Save.

Add Document opens and the image file is listed under Files.  Tap Add Page to add additional pages.

11. Tap Done.
