If you have forgotten your FMH username and need to recover it, please follow the steps below. Note: this feature is not yet available on the mobile app, but can be accessed in the mobile browser.
From the sign in page, click on Forgot your username.
You will be prompted to enter your email address, phone number, name, and date of birth. Click on the reCaptcha button to prove you are not a robot, and click Continue when you are finished entering your information.
You will be sent a verification code to recover your username. Choose whether the code will be sent to your email or texted to your mobile device. Click Continue.
Once you receive your verification code, enter it in the field shown below and click Continue. You can also have the verification code re-sent if needed. If you did not receive a verification code, please contact your provider's support team for help. Note: You will have 3 attempts to recover your username in a 24 hour period. If you exceed your allowed attempts, contact support.
Once the verification code is entered, you will be shown your username. You are also given the opportunity to recover your password if needed. Click Continue.
