FMH Password Reset Assistance

If you have forgotten your FMH Secure Login Password and need to reset it, please follow the steps below.

From the username/password screen, click on the 'Forgot your Password?' link



Enter your username in the box and select "Reset".



Verification email will be sent to your notification email inbox:

If you did input a valid username, then a Your Verification Code email will be sent to the address associated with that specific username.  See below for a reference of what that email looks like. 




After returning to the web browser and entering the verification code, you will be taken to the Password Reset screen.  From here you will input a New Password and confirm it.  Be sure to follow the criteria for creating strong passwords, as they are required by FollowMyHealth.  Any errors in the password reset will be Missing a green check mark next to the missing criteria.



The New Password and Confirm Password match when the Update Password screen appears "clean".



An email will also be sent to confirm the successful reset. 



Once updated, return to the FollowMyHealth login screen to access your portal.
