FMH Password Reset Assistance

If you have forgotten your FMH Secure Login Password and need to reset it, please follow the steps below.

From the username/password screen, click on the 'Forgot your Password?' link



Enter your username in the box and select "Reset".



A notice of a successful request will then populate.  Please note the "if the username was recognized" statement.



If you did input a valid username, then a Password Reset Request email will be sent to the address associated with that specific username.  See below for a reference of what that email looks like.  Click on the "Reset Password" link.



NOTE:  If you select "Report unwanted password reset", all attempts to reset the password will be blocked for 24 hours.  Click here for more information on why this may be happening.

After selecting "Reset Password", you will be taken to the Password Reset screen.  From here you will input a New Password and confirm it.  Be sure to follow the criteria for creating strong passwords, as they are required by FollowMyHealth.  Any errors in the password reset will be highlighted in red



The New Password and Confirm Password match when the Update Password screen appears "clean".



Select "Save" and then a Password Updated screen will appear.  An email will also be sent to confirm the successful reset. 



Once updated, return to the FollowMyHealth login screen to access your portal.
