My Medical Information is Wrong or Missing

Please be aware that FollowMyHealth® reflects your medical information exactly as it appears on your health records with your medical organization. The medical information released to you in FollowMyHealth is under the discretion and control of your medical organization. 

You can also verify that the clinic settings in your Patient Portal are not disabled for data to flow.  Go to “My Account > My Connections”.  Here, you will click on the Hospitals or Clinics icon and then click on the clinics settings.

If your FollowMyHealth account has incorrect or missing information, and you have verified that your clinic settings are enabled, please contact your provider or medical clinic.
