FMH Secure Login Registration

Please Note:  If you are starting completely from scratch, without an invitation, click on the “I Need to Sign Up” button. Fill out the corresponding form, and then select “Confirm and Continue”.

When creating a new FollowMyHealth account, you will have the option to select the FMH Secure Login method as a way to get into your account.  This icon looks like a green shield.  In doing so, you will be asked to create a unique username and password, which you will use as your UHR credentials.   Please see the instructions below for creating an FMH Secure username/password. 

Click the FMH icon. A new window will then open prompting you to create your username and password.  It is recommended that you use your email address as your username.

If you share an email address, or prefer to use a different username, you can create a username of your choice.

Once the username has been created, you will then move to the password creation section of the page.  Create your password according to the specified list to the right.  As they are completed, the list will turn green with check marks next to each item.  Any item still in red means the criteria has not yet been met.

FMH Secure Login enforces strong passwords, so the registration screen cannot be completed unless the password chosen has at least one alpha, one numeric, and one special character, such as: !@#$%^)&*-(. Your password should be no less than 8 characters in length. Your password must be confirmed to complete the registration and must match the original.

Once the list is completely green, select “Confirm and Continue” at the bottom of the page.

Note: You will receive a verification email once you have created your account. It will be come from noreply@followmyhealth.com and have the subject, “Success! Your username was created.”  FollowMyHealth does not store passwords for patient privacy reasons. Do not delete or misplace this email, you will need your username to recover your password.

In the event the username confirmation email is lost or deleted, please complete Username Recovery or contact your patient support team.  By providing your name and date of birth, patient support may be able to recover your username.
