My medical information is wrong or missing.

Please be aware that FollowMyHealth® reflects your medical information exactly as it appears on your health records with your medical organization. The medical information released to you in FollowMyHealth is under the discretion and control of your medical organization.

You can also verify that the clinic settings in your Patient Portal are not disabled for data to flow.  Go to My Account > My Connections > Visualize My Connections.  Here, you will click on the hospital or clinic and then to the Clinic Settings.  Data flow can be enabled/disabled by clicking on the On/Off toggle buttons or by clicking on the service icons (Appointments, News, Personal Health Record, etc.).

If your FollowMyHealth Universal Health Record has incorrect or missing information, and you have verified that your clinic settings are enabled, please contact your provider or medical clinic directly.
